Message program
Balance Hormone Center, LLC (“Balance Hormone Center”, “we”, “us”) offers a text
message (SMS) program for our patients. It covers messages about your own
appointments and secure links to your own health information. The program is
available to patients who have given us their mobile number and have
affirmatively consented to receive these messages.
What we send
Two kinds of message travel on this program.
Messages about your appointments:
- reminders before a scheduled visit;
- confirmation when a visit is booked;
- notice when a visit is rescheduled or cancelled.
Messages carrying a secure link, to a page where you can:
- complete a form we have asked you to fill in before or after a visit; or
- view a summary of a visit you have had with us.
Opening a link asks you to confirm your date of birth before anything is
shown. The text message itself contains no lab results, diagnoses,
medications, or other clinical information — only who the message is from
and the link.
We do not send marketing or promotional text messages through this program.
How you opt in
You opt in during Balance Hormone Center patient intake, by selecting a
consent checkbox that is unchecked by default. The disclosure shown next to it
identifies Balance Hormone Center, describes the messages, and links to this
page and to our Privacy Policy. That checkbox is the only way to join this
program.
We ask for a phone number when we open your chart because we need one to send
a prescription to a pharmacy and to reach you about your care. Giving us your
number does not enroll you in this text message program. Enrolling is a
separate choice, and the form submits whether or not you make it.
You can also reply START to a message from us to re-enroll after
opting out.
Consent to receive text messages is not a condition of purchase or of
treatment.
You will receive the same care whether or not you enroll, and you may withdraw
consent at any time. If you do not enroll, we will call you about your
appointments, and send your forms and visit summaries by email or give you a
printed copy instead.
Message frequency and cost
Message frequency varies — it depends on how often you
visit us, how many appointments you book or change, and how many forms we send
you.
Message and data rates may apply. Balance Hormone Center does
not charge for these messages, but your mobile carrier may charge you for
messages or data according to your plan. Contact your carrier for details about
your plan.
How to stop
Reply STOP to any message from us to cancel. You will receive a
single confirmation that you have been unsubscribed, and we will not send you
further messages in this program. You can also call or email us using the
details below and ask us to remove you.

Links in these messages
Every link we send in this program points to a page on a Balance Hormone Center
domain. We will never ask you for a password, a payment card, or a Social
Security number by text message, and we will never ask you to install anything.
If a message claiming to be from us asks for any of those, it is not from us
— call the number above.
Each link is personal to you and expires. Please do not forward it.
Carriers and delivery
Supported carriers include AT&T, Verizon Wireless, T-Mobile, and most other
U.S. carriers. Carriers are not liable for delayed or undelivered messages.
Message delivery depends on your carrier and your device, and we cannot
guarantee that any individual message will be delivered. Do not rely on text
messages for urgent matters.
Emergencies
This text message program is not for emergencies or urgent medical needs. It is
not monitored continuously and it is not a way to reach a clinician. If you have
a medical emergency, call 911. For urgent clinical questions, call us at

Privacy
Our handling of your mobile number and your consent is described in our
Privacy Policy. In particular: no mobile
information will be shared with third parties or affiliates for marketing or
promotional purposes.
Changes to these terms
We may update these SMS Terms & Conditions. Changes will be posted on this
page with a revised “Last updated” date. Your continued participation in the
program after a change constitutes acceptance of the updated terms.
