About this message program
Balance Hormone Center sends transactional text messages to existing patients who have asked to receive them. Two kinds of message travel on this program:
- Appointment messages — reminders, confirmations, and notices when an appointment is rescheduled or cancelled.
- Secure links — a link to a form we have asked you to complete, or to a summary of a visit you have had with us. A message carrying a link contains the link and nothing clinical, and opening it asks you to confirm your date of birth before anything is shown.
We do not send marketing or promotional text messages. Messages go only to the mobile number you provided to us.
How patients opt in
The Balance Hormone Center new patient intake form is the only way to enroll. There is no verbal, paper, or in-person enrollment. The form is completed electronically by the patient and is issued to a named patient, so it is not publicly reachable — the screenshots below show the two relevant steps exactly as a patient sees them.
Step 1 of 6 — where the phone number is collected
A phone number is required to open a chart, because we need it for electronic prescribing and to reach you about your care. Providing your phone number does not enroll you in text messages.

Step 6 of 6 — the text message consent checkbox
Enrollment is a separate, optional step. The consent checkbox is unchecked by default and carries no “Required” marker, while other fields on the same screen are marked required. The form submits successfully with the box left unticked.

The checkbox reads:
I agree to receive text messages (SMS) from Balance Hormone Center at the phone number I provided.
The disclosure shown directly above the checkbox reads:
Balance Hormone Center can send you text messages at the phone number you provided earlier in this form: reminders, confirmations, and notices when an appointment is rescheduled or cancelled, and a secure link to a form we ask you to complete or to a summary of your visit. A message with a link contains the link only — no clinical information — and opening it asks for your date of birth. Message frequency varies. Message and data rates may apply. Reply STOP to opt out or HELP for help at any time. Consent is not a condition of purchase or of treatment. No mobile information will be shared with third parties or affiliates for marketing or promotional purposes.
